Release & Authorization
I certify that the information provided herein is true, correct, and complete to the best of my knowledge. I understand that false, misleading, or incomplete information provided during the application process may result in disqualification from consideration for employment or termination if already employed. I authorize TESSA to conduct a thorough investigation of all information provided by me in this application and during the application process. I voluntarily and knowingly authorize any present employer or supervisor, past employer or supervisor, college, university or other institution of learning, administrator, law enforcement agency, state agency, federal agency, finance bureau/office, credit bureau, collection agency, private business, military branch or the National Personnel Records Center, personal references, and/or other persons, to give records or information they may have concerning my criminal history, motor vehicle history, earnings history, credit history, character, volunteer/intern history, employment records, or any other information requested to TESSA. I voluntarily and knowingly unconditionally release any named or unnamed informant from any and all liability resulting from the furnishing of this information. This authorization shall be valid one year from the date signed and a photographic or faxed copy of the authorization shall be as valid as the original. If hired or selected as a volunteer/intern, I understand that my employment or volunteer efforts/internship is contingent on the results of the background checks, is for no specific period, and may be terminated at any time by TESSA or me. I also understand that if hired or selected as a volunteer/intern I am required to abide by all rules and regulations of TESSA. This application is not an employment agreement. I understand that TESSA is an at-will employer and may terminate my employment or volunteer efforts/internship at any time, with or without cause and with or without notice. I also understand that no supervisor, manager or other representative other than the President of the Board of Directors and the Executive Director has any authority to enter into any promises or commitments contrary to the foregoing statements, including making any agreement for employment for any specified period of time.
